What does Fast Track CHC include?
Fast Track CHC is an emergency pathway within the NHS Continuing Healthcare framework. Unlike standard CHC, it does not require the full Decision Support Tool (DST) assessment. A clinician can trigger it when someone has a rapidly deteriorating condition, is approaching end of life, or has an urgent need for a care package that cannot wait for the standard process.
When Fast Track is correctly applied, NHS funding can be in place within days - not weeks. The problem is that it is often not offered to families who qualify, or it is refused by the Integrated Care Board (ICB) without adequate clinical justification.
Who is this for?
This service is for you if:
Your loved one's condition is deteriorating quickly and they need care arranged urgently
A clinician has suggested Fast Track CHC but the ICB has refused or delayed it
Fast Track funding was previously in place but has been withdrawn
You believe your loved one is approaching end of life and no one has mentioned this pathway
You are being asked to fund urgent care privately while waiting for a decision
What We Do
When SG67 takes on a case, we manage the process from start to finish:
Review the clinical evidence and assess whether Fast Track criteria are met
Escalate urgently to the ICB if Fast Track has not been offered or has been refused
Prepare and submit clinical evidence to support the Fast Track application
Challenge any decision to refuse or delay Fast Track on clinical grounds
Liaise directly with healthcare professionals, care providers, and the ICB on your behalf
Work to have funding in place as quickly as possible - often within days
When Fast Track CHC is secured, the NHS funds the full cost of your loved one's care package immediately - removing the financial and logistical burden from your family at the most difficult time.
