What does Care Planning Support include?

A care plan is the formal document that records an individual's care needs, preferences, and the agreed care package designed to meet them. For CHC purposes, the quality of the care plan can be decisive - a well-written plan that accurately documents clinical complexity strengthens an application; a generic or inadequate plan can result in needs being under-recognised. SG67's care planning support ensures the document works in the person's favour.

Care plans are also relevant outside of the CHC process - for individuals receiving care at home, in care homes, or in any setting where the level and nature of care needs to be formally agreed.
