The CHC Checklist is Stage 1 of the funding process. Many families are incorrectly screened out here. We prepare you, attend with you, and challenge any domain that's been under-scored.
What does CHC Checklist Support (Tool 11) include?
The CHC Checklist is a screening tool covering 11 care domains including behaviour, cognition, communication, continence, skin integrity, mobility, nutrition, breathing, and psychological needs. It must be completed by a healthcare professional - but the quality and accuracy of completion varies significantly. A person should progress to full DST assessment if two or more domains score "severe" or one domain scores "priority".
The consequences of being incorrectly screened out at Checklist stage are significant. The family continues paying for care that the NHS should fund - often thousands of pounds per month - while the person who may qualify never reaches the full assessment.

What We Do
When SG67 takes on a case, we manage the process from start to finish:
Review all available medical and care records in advance of the Checklist
Prepare a detailed clinical summary of the person's needs across all relevant domains
Attend the Checklist meeting as a registered nurse advocate
Challenge any domain score that does not accurately reflect the person's needs
Ensure the assessor considers the nature, intensity, complexity, and unpredictability of needs - not just diagnosis
Advise on next steps if the Checklist outcome is disputed
The aim is to ensure your loved one progresses to a full DST assessment - the stage at which CHC eligibility can be formally confirmed and NHS funding awarded.
